What is a forex markup?

It is the fee a bank adds when you pay in a foreign currency, commonly 1% to 3.5% of the amount. Some travel cards charge less, or nothing.

Are lounge visits free?

On most cards they are free up to a set number a year, and some cards need a minimum spend in the previous quarter to unlock them.

Do I need a travel card if I travel once a year?

Probably not. A card with no annual fee and a low forex markup usually serves an occasional traveller better.
